Central Health Medicare Plan’s Healthy Actions Rewards Program was made just for our members. We want to support you on your health journey! See your doctor, complete your healthy actions, and earn rewards with Central Health.

What Are Healthy Actions?

Healthy actions are routine screenings you complete with your doctor. These visits are important to keep you healthy or catch health problems early. They help your doctor come up with a care plan for you.

Follow these simple steps to earn your rewards:

  1. Complete your healthy actions with your doctor between 1/1/2026 and 12/31/2026.
  2. Claim your rewards and let us know what screenings you completed by 1/31/2027.
  3. Use your rewards by 3/31/2027.

Complete these screenings to earn your rewards:

SCREENING

DESCRIPTION

Annual Wellness Visit

Yearly in-person check-up with your doctor or in-home visit. Talk to your doctor about your care plan and ask any questions you may have.

Colon Cancer Screening:

Screening to check for signs of colon cancer in adults ages 45-75.

Eligible screenings include:

  • Fecal occult blood test – This test can be done at home and looks for traces of blood in a stool sample.
  • FIT-DNA Test – This is a stool-based screening that can help your doctor find possible cancerous changes to your colon.
  • Sigmoidoscopy – This test examines the lower part of your large intestine. Your doctor will look for signs of colon cancer.
  • Colonoscopy – This test looks at your entire large intestine for signs of colon cancer. Your doctor may take small samples from areas of concern.

Breast Cancer Screening:

Mammogram to check for signs of breast cancer in women ages 40-74.

 

A mammogram is an x-ray of your breast. The test should only take a few minutes. Your doctor will use a special x-ray that will compress each breast. This may cause some discomfort but should not hurt.

Flu Shot

Yearly vaccine to help lower your chances of getting the flu.

Diabetes Screenings

Yearly diabetes screenings for adults ages 18-75 with diabetes.
*You must complete all 3 screenings to claim your reward.

 

A1c Test:

Blood test to check your average blood sugar levels.

Diabetic Eye Exam:

Yearly exam with your eye doctor to check your eyes for signs of damage caused by diabetes.

Diabetic Kidney Health Test:

A urine and blood test to check your kidney health.

Complete Health Screenings from Home

Want to complete some screenings from the comfort of your home? Central Health can mail you a home lab kit. The lab kits can check:

  • Your A1c level
  • Your kidney health
  • For signs of colon cancer

What information do I need to claim my rewards?

For each screening you complete, we will need to know:

  1. The screening you completed
  2. Date your screening was completed
  3. Where you completed the screening:
    • Office name
    • Doctor’s name
    • Phone number

How Do I Claim My Rewards?

You must complete your health screenings before you claim your rewards. You can claim your rewards by:

  1. Nations Benefits Portal

    Visit CentralHealthPlan.NationsBenefits.com and enter your appointment information after your visit.

     

  2. Phone

    Call the Wellness Team. Let them know:

    • When you saw your doctor
    • What screenings you completed
    • Your doctor’s name and phone number

     

  3. Mail

    Print and fill out the form located on the Central Health Medicare Plan Member Portal.
    Mail the form back to Central Health in an envelope with a stamp to:

    Visit your Central Health Medicare Plan Member Portal for details on mailing in your form.

 

How Can I Use My Rewards?

Your rewards will be loaded onto your Benefits Mastercard Prepaid Card. You can use your rewards to buy healthy food and produce. For a list of products, click here. You can visit CentralHealthPlan.NationsBenefits.com to find stores that are part of the program.

 

What Happens to my Rewards if I Disenroll?

You must be an active Central Health member to claim your rewards. You will lose your unspent rewards if you disenroll with Central Health.

Central Health’s reward program is subject to change without notice based on guidance from the Centers for Medicare and Medicaid services (CMS) and the state.

Members must complete the eligible service(s) before claiming the reward. By taking part in the program, members understand that Central Health may call to ask for details about the completed visit.

CHP Central Health Medicare Plan is an HMO/HMO SNP plan with a Medicare contract. Enrollment depends on contract renewal.
