Central Health plans help fill in the gaps and cover the costs that aren’t part of your Original Medicare Part A & Part B programs. You’re already eligible for one of our plans if:

You live in our Geographic Service Area 1

and

You are a United States citizen (or lawfully present in the US)

and

You have both Medicare &

Do You Have Special Needs?

If you’ve been diagnosed with Diabetes, Cardiovascular Disorders and/or Chronic Heart Failure, you’re eligible to join the Central Health Focus Plan (HMO C-SNP).

If you’re enrolled in , you may be eligible for our Medi-Medi Plan HMO D-SNP or Ventura Medi-Medi Plan HMO D-SNP.

Enrollment Periods

If you meet the eligibility requirements above, you can enroll in one of our plans:

  • Between October 15–December 7 (for coverage starting January 1)
  • Within 3 months of first getting Medicare Part A & Part B
  • In other special situations when you’re allowed to join or switch plans

Important Information

To enroll in any Central Health Plan, you will need to provide your current Medicare Member Number (on your Medicare card) and your permanent address and phone number. Please note that you must complete all items in sections marked “Required.” All other sections are optional and you cannot be denied coverage because you don’t fill those out.

Once you’re enrolled, we will send you a bill for the plan’s premium. You may also choose to have your premium payments deducted from your bank account or your monthly Social Security (or Railroad Retirement Board) benefit.
