- You may be eligible if: you are an older adult and/or an adult with a disability and eligible for Medicaid but not Medicare, you or your child is eligible for full Medicaid benefits.
- To enroll with Molina Healthcare HealthChoice Illinois, you also need to live in our service area.
- If you are eligible, you would have received information from Illinois Client Enrollment Services.
