There are federal and state laws that protect the privacy of your medical records and personal health information. We keep your personal health information private as protected under these laws. Any personal information that you give us when you enroll in this plan is protected. We will make sure that unauthorized people do not see or change your records. Generally, we must get written permission from you (or from someone you have given legal power to make decisions for you) before we can give your health information to anyone who is not providing your care or paying for your care.

Central Health Plan of California (CHPC) takes your privacy seriously. This privacy policy ("Policy") explains how we may collect, use, or disclose your personal information or other data through the time that you are a member of CHPC as well as through www.centralhealthplan.com (the “Site”). We reserve the right to change the provisions of this Policy at any time. We will alert you that changes have been made by indicating on the Policy the date it was last updated. We encourage you to review this Policy from time to time to make sure that you understand how any personal information you provide will be used.


What Is Personal Information?

As used in this Policy, the term "personal information" means information that can specifically identify you (such as your name, address, telephone number, mobile number or e-mail address), or other information about you that is directly linked to personally identifiable information. Personal information does not include data we collect about the use of the Site or about a group or category of services or users, from which individual identities or other personal information has been removed. This Policy in no way restricts or limits our collection and use of aggregate information.


What Information Do We Collect?

These third parties have access to your information only to perform these tasks on our behalf.

What Steps Do We Take To Protect Your Information Online?

CHPC does not knowingly collect any personal information from anyone under the age of 13. If we learn that we have collected any personal information from a child under the age of 13 without verifiable parental consent, we will delete that information from our database as quickly as possible.
